Quality of life fixes

Fixes made to improve basic game play:

- Fixed most cases of not being teleported to arena, death plane added if you still fall through

- Can skip intro with 'space' or 'r'

- Depth of player attack more visible

- Enemies cannot kill you, and other small animation changes

Misc. updates:

- Death texture set to proper file

- Intro song will not loop/start at end
